Frontonasal Dysplasia

From what I could find on the internet, this is a picture taken of a wax model of a guy named Bill Dax (Not Bill Durks) that is apparently displayed at a place called the Muzeum lidských zvláštností.
The Czech to English translation being "The Museum of Human Traits".

Info states that Dax committed suicide at the age of 46.
